CONSENT FORM TO ALLOW USE OF INFORMATION OBTAINED THROUGH PARTICIPATION IN RESEARCH IN FUTURE SPEECHES, CONFERENCES OR OTHER PUBLIC EVENTS.
This is to state that I agree to allow the use of the information and opinions I disclose through the research being conducted by George Oblin, Ph.D. candidate in Communication Studies at Concordia University, in future speeches, conferences, or other public events.
A. PURPOSE
I have been informed that the purpose of the research is to obtain information and insights into the Agreement Concerning a New Relationship Between Le Gouvernement Du Quebec and the Crees of Quebec (commonly known as the Paix des Braves Agrreement) and Cree responses to that agreement.
B. PROCEDURES
The research will be conducted both within and outside the Cree communities of northern Quebec.  Interviewees will be asked to share information, thoughts and insights about the Agreement and its significance in relation to Cree society.  Some interviews will be conducted one-on-one with the researcher for a mutually agreed upon duration of time and some information will be sought via mailed questionnaires, e-mail, group interviews or telephone interviews.  Each participant will have the option of participating in this research either in a confidential or non-confidential capacity.
C. CONDITIONS OF PARTICIPATION
